As a critical component of our re-entry plan, parents are required to print, initial, sign and submit the form below as their child begins in-person instruction for the week. A hard copy will be collected prior to your child entering the building.
If any information changes throughout the course of the week, you must contact the school nurse immediately. Your assistance is greatly appreciated.
